Illinois Witness Slip Instructions

If you are an IL resident 13 or older, take these steps to quickly voice your opinion:

  1. Click on the link above.
  2. Fill in your information. Put "Self" in three fields:
    • Firm/Business
    • Title
    • Persons, groups firms represented in this appearance
  3. Under Position, click either PROPONENT or OPPONENT (be sure to click the correct one).
  4. Under Testimony, click "Record Of Appearance Only"
  5. Click "I Agree to the ILGA Terms of Agreement"
  6. Click button Create(Slip)

If you create an account you can save your information for future witness slips.
